Question:

Dear Doctor, I send extract of three reports of mamograms with recent health check details done at Cancer Hospital with a request for seeking your valuable guidance. 1.Mammogram dated 31st January 2003 Breast Parenchyma appear nodular on both sides. No evidence of skin thickening or nipple retraction seen.Subareolar tissue is unremarkable. No micro-calcification seen in both breast on present study. On sonofraphy examination,small cysts are seen in left breast. The biggest cyst is about 12x5 mms size seen in superior and lateral quadrant. No definite mass seen in right breast on sonohraphic study. COMMENTS : Nodular breast parenchyma on both side. Small cysts are seen in left breast on sonographic study suggestive of benign lesions. 2. Mammogram dated 14-11-2003 Breast Parenchyma appear nodular on both sides. Urine- Normal 3.Hbs Ag : Negative by Elisa method 4. Pap Smear : Satisfactory for eveluation, obscuring inflammation present. Negative for intraephitelial lesion or malignancy 5.Lipid Profile : S Cholestrol: 186 S Trigliceride 60 HDL 67 VLDL 12 LDL 107 HDL/Chol Ratio 0.36 6. X Ray Chest Normal Bilateral memogram : Both breasts show dense parenchymal pattern which on USG has features suggestive of fibrocystic changes. Two hypoechoic lesions of size 13x4 mm and 10x4 respectively in the left central outer quadrant. Possibility of two fibroadenomal appears likely. No evidence of skin thickening or nipple retraction or microcalcification noted in either breats. USG whole Abdomen Normal. FSH 63.90 - LH 27.88 TSH 1.6 FBS PPBS Normal HBP on treatment Betaloc 50 mg twice and Losar-H ½ once a day Her CAG in October 2003 absolutely normal. Sir, She has occasional pain on the left side of the breast Looking to the previous 3 reports of mamograms and recent reports, do you find any abnormality and whether, surgical intervention at this stage for removal of fibroadenoma is advisable and do you find any warning signals in the foregoing. Answer:

Based on what has been sent, it appears that your wife has benign cyst and perhaps a fibroadenoma (which is a benign tumor of the breast). Unless there is a change in appearance over time, which there is not on the reports, surgery is not indicated. However, if your wife's symptoms or xray change then she will need to be reevaluated.
