Progesterone Dose During Pregnancy

Question:
Is 200mg of progesterone vaginally 3 times daily an excessive dose for maintenance of pregnancy? Would it matter if the person had factor v leiden?
Answer:
This is a difficult one. The American College of Obstetrics and Gynecology endorses a protocol of progesterone 100 mg vaginal suppositories once daily or progesterone 250 mg IM once weekly in women with a history of preterm delivery. Your condition may require a higher dosage. You should ask your doctor if he/she has a primary reference for the dose he/she is using. Perhaps this high dose is just for a short period of time, like a loading dose, and the plan is to lower it to once daily at a later date.
